Autoimmune hemolytic anemia associated with drugs
drugs may induce hemolysis by one of two mechanisms
1- Innocent bystander :
the patient develops antibody against the drug which forms a complex with it , which then attaches passively to the red cell membrane . Complement is then boud and causes destruction of the red cell . Example : sulfer , sulphonylurea and chlorpropramide .
AB+drug C (RBS).
2- The drug as a hapten :
The drug binds firmly to the red cell membrane and an antibody is then formed with specificity for the drug – membrane complex . Treatment should involve withdrawal of the offending drug if additional treatment is required it should follow the same lines as those for warm antibody AIHA .
